This answer is incorrect. Although this patient is at risk for immune reconstitution inflammatory syndrome due to recent initiation of antiretroviral therapy, one would have expected cough, hemoptysis, or hypoxemia if this pulmonary immune reconstitution was causing his clinical illness.
