What We Do/Who We See :

Obesity/ Wt Loss counseling
Diabetes- Type 1 and 2,
  Newly diagnosed very important
  Long-term diabetes--- often need refresher… what’s their HgbA1c?
  We often work in tandem with Pharmacists (food/BG/activity logs)
Metabolic Syndrome
Osteoporosis (esp. Calcium & vit. D deficiency with this population)
Hypertension
Hyper-/Dyslipidemia
COPD
Malnutrition/unintentional wt loss ( ** note below)
Hepatic Disease
GI disorders

   ** Pt’s with Malnutrition/Weight loss (SUPPLEMENT USE e.g. Ensure, Boost):     Referral to a Nutritionist is your best option . We can review some other strategies for weight gain and will assess if they meet DSHS eligibility guidelines for supplements.

Referral Process:

Please complete a “Request for Consultation or Referral” form for every pt. ( with LABEL)

Classes available :
   Diabetes: 2 sessions + follow-up, repeated each month (English)
     Other languages : Span, Viet, Amharic, Somali, Cambod--- offered periodically
  Weight Management: 6- or 12-week class; Support Group weekly year-round (English)
  Heart Healthy: 3 class series q month (English)
  Walking Group/Class: 3 class series quarterly (MD, RD, PT). Pedometer given. (English)
