UW Student Insurance Offers Free HVP Vaccinations
The UW Student Insurance Office, a unit of Student Life, is offering free vaccinations against the human papilloma virus (HPV) to students/spouses/domestic partners who are covered by the UW Accident and Sickness Insurance Plan.
The vaccinations are available only at Hall Health Primary Care Center: http://depts.washington.edu/hhpccweb/ [ see map ]. There will be a $35 administration fee (effective 7/1/08) charged for each of the three vaccinations but the $160 fee per vaccination will be provided at no cost to the student.
What is HPV?
HPV is a virus that can cause cervical cancer and genital warts. It is spread through sexual contact: http://depts.washington.edu/hhpccweb/index.php?ClinicID=17.
Who is eligible?
Students, spouses, or domestic partners who are covered by the UW student insurance plan, between the ages of 11 and 26, are eligible for the series of three vaccinations if they are enrolled in the voluntary Accident and Sickness Insurance Plan.
When does the program start?
On March 24, 2008. Only insured students will be able to complete the application materials. Eligible spouses and/or domestic partners cannot apply directly but must have their application completed by the student (i.e., the husband or partner).
How do I apply for the voucher?

Within 24 hours, the student will receive an email voucher notification. The person receiving the vaccination must present a printed copy of this email at the Immunization Clinic at Hall Health Primary Care Center.
A separate voucher must be requested for each of the three vaccinations. The voucher for the second vaccination may be requested 60 or more days after the first voucher is issued. The voucher for the third vaccination may be requested 120+ days following the second voucher.

How do I pay the $35 administration fee?
The administration fee of $35 per injection can be paid by cash, check, Visa or Mastercard. This fee must be paid at the Patient Services Center at Hall Health at the time the vaccination is given.
