Building Hours
Posted building hours for the Health Sciences / WH Foege Building are Monday thru Friday from 7 am to 6 pm.
Access Cards

Only one access card may be active for an individual at a time.
Personnel who have a Health Sciences access card must have an approved photo identification (ID). HS will supply
a photo ID, free of charge, or accept a photo ID from an affiliated institution (e.g., UWMC, HMC, CHRMC, SCCA,
FHCRC) or other UW department (e.g., Facilities Services).
Access cards are University of Washington property and must be returned to the issuing department upon termination
of appointment, employment, or registration.

New Cards
Notice: effective July 1, 2011: assignment of a new card costs $13.
- Fill out the Health Sciences
Photo ID and Access Card Request Form 20 (updated 4/2012).
- The Building Access Coordinator (BAC) should
sign the form and provide a budget number.
- If a HS photo ID must be made, the individual must bring Form
20 (pdf*) to T283 during posted photo session hours, as well as photo ID for verification of identity.
- If this is a new card and a photo is already on file, it can be transferred to the new card. The individual
does not need to have the photo taken again, unless he/she would like to.
- If the person does not need a HS photo, the BAC may mail or fax the Form
20 (pdf*) to: Health Sciences Building Management Box 357175 Fax: 616-3116
- If the individual does not come in for a photo, the BAC will be notified when the card is ready for pick-up
in Room T283. (Access cards are not mailed.)
- If the individual takes their access card, verification will be sent to the BAC of the assignment of a new
card and its number.

Reprogramming Card
Notice: effective July 1, 2011: Re-issuing a previously used card costs $8.
- To reprogram/re-issue a card, fill out the Health
Sciences Photo ID and Access Card Request Form 20 (pdf*)
- Please indicate on the Form
20 (pdf*) the card number you want re-assigned.
- Have the BAC sign the form and provide a budget number.
- The individual must bring Form
20 (pdf*) to T283 during posted photo session hours, as well as a photo ID for verification of identity.
- If the person does not need a HS photo ID, the BAC may mail or fax the Form
20 (pdf*) to: Health Sciences Building Management Box 357175 Fax: 616-3116
- If the individual does not come in for a photo, the BAC will be notified when the card is ready for pick-up

Deactivation of Cards
When a cardholder's status changes, (terminates, moves off-site, transfers to a new department, etc.), please
report this information to Building Management for deactivation of his/her card.
- Email the cardholder's name and card number to hsbuild@u.washington.edu.
- Deactivated cards should be kept by the BAC for reactivation.
- There is no charge for deactivations.
