Access Cards
Access cards are University of Washington property and must be returned to the issuing department upon termination of appointment, employment and/or registration.
An appropriate photo ID is required for anyone requesting a HS access card. Acceptable photo IDs are HS, UWMC, HMC, CHRMC, SCCA, FHCRC, UW Facilities Services. Get a Photo ID.
New Cards
The assignment of a new card costs $10.
- Fill out the Health Sciences Photo ID and Access Card Request Form 20 (87K PDF*)
- The Building Access Coordinator (BAC) should sign the form and provide a budget number.
- If a HS photo ID must be made, the individual must bring form 20 to T283 during posted photo session hours, as well as photo id for verification of identity.
- If this is a new card and a photo is already on file, it can be transferred to the new card. The individual does not need to have the photo taken again, unless he/she would like it.
- If the person does need a HS photo, the BAC may mail or fax the Form 20 to:

Fax: 616-3116 - If the individual does not come in for a photo, the BAC will be notified when the card is ready for pick-up in Room T283. (Access cards are not mailed.)
- If the individual already collect the access card, verification will be sent to the BAC of the assignment of a new card and it's number.

Reprogramming Card
Re-issuing a previously used card costs $5.
- To reprogram/re-issue a card, fill out the Health Sciences Photo ID and Access Card Request form 20 (87K PDF*)
- Please note the card number you want re-assigned.
- Have the BAC sign the form and provide a budget number.
- If the person needs a HS photo ID, he/she must have a new photo taken, as an existing photo cannot be transferred to a re-issued card.
- The individual must bring form 20 to T283 during posted photo session hours, as well as photo id for verification of identity.
- If the person does not need a HS photo ID, the BAC may mail or fax the Form 20 to:

Deactivation of Cards
When a cardholder's status changes, i.e., terminates, moves off-site, transfers to a new department, etc., please report this information to Building Management for deactivation of their card.
- Email the cardholder's name and card number to hsbuild@u.washington.edu.
- Deactivated cards should be kept by the BAC for reactivation.
- There is no charge for deactivations.
