Getting a New Badge
- Fill out the Health Sciences Photo ID and Access Card Request Form 20 [pdf*] updated 6/2009.
- Have the form signed by an authorized Building Access Coordinator from the individual's department.
- Photo IDs are taken in Room T283 (entry through T283) during regularly scheduled sessions. Bring Photo ID for verification of identity.
- A special appointment may also be made, if open photo session times are inconvenient. Call Building Management at 685-0143 to make an appointment.
- Each person obtaining a photo ID receives their initial badge, plus a clip or lanyard, free of charge.
- This process takes about 5 minutes per person.
- The person signs the Form 20 that he/she has received the photo ID.
Getting a Replacement Badge
- Personnel must report lost, stolen, or destroyed badges to their department Building Access Coordinator immediately.
- Fax or mail the Health Sciences Photo ID and Access Card Request form 20 [pdf*] to:

Fax:616-3116 - We will produce the replacement badge using the photo on file ($6 charge).
- If an access card is requested, it will be programmed at the same time (no charge for photo id when done at same time).
- A clip or lanyard is provided as a courtesy with each replacement ID badge.
- The Building Access Coordinator will be contacted when the ID badge and/or access card is ready for pick-up in T283.
Changes in Information
- If you have an HS photo ID and lose your access card, both items must be replaced. This is necessary because the access care number is printed on the photo ID. The photo ID is free of charge, but the access card coss $11.
- When a person changes departments, they must have a new photo ID made, because the department name is listed on the badge (no charge).
- Name changes require a new photo ID (no charge).
Building Access Cards
- If you need to enter Health Sciences / Foege outside of normal business hours, you will also need an access card.
