Copyright Clearance Form (56K PDF*)
This form is used by clients to identify the copyright status of any image used in Creative Services projects. It is intended for images used outside of the classroom environment, for example, event posters or web sites.
Release Form (51K PDF*)
This form is for model and talent releases of image, likeness, and audio. This is only to be used when there is no compensation IN ANY FORM. If there is to be compensation IN ANY FORM, it is the responsibility of the client to aquire releases, and assign rights of use through the use of a Copyright Clearance Form.
