Application Process
The instructions, forms and checklists below provide the information you will need to apply to the Extended MPH Degree or Certificate Program
The Extended MPH Degree Program uses the UW Graduate School online application for both the degree and certificate programs. After you create a user profile and select your program, you'll be given specific instructions for how to complete each element of the application. Then, once you've completed each required element, you'll be able to pay the application fee online and submit your application.
Applications are considered on a rolling-basis and cohorts are limited to 30 students. Applicants who submit all materials by the first business day following February 15th will receive priority review.
Completed applications received after that date will be considered on a space-available basis only.
(International applicants should submit original transcripts, TOEFL, and degree reports to the Graduate School as close as possible to November 1 of the year prior to admission.)

If you've never used the online application before, start where it says New Applicants - Start Here near the upper right corner of that page.
The statement of purpose should include a description of the applicant's professional background, career goals, and the educational objectives which he/she expects to meet through the program. An applicant should be sure to include a description of his or her work experience in health care or related fields. The admissions committee places particular importance on well-specified and realistic career objectives.
Personal Statement (optional):
We welcome students who have varied cultural experiences, and who will therefore contribute to the intellectual and social enrichment of the academic programs. If you wish to have these factors included in the review of your application, please provide a statement concerning your personal history, family background, and influences on your intellectual development. This statement should include cultural and educational opportunities (or the lack thereof), social and economic disadvantages that you may have had to overcome, and the ways in which these experiences have shaped you and informed your academic and research interests and your personal and career goals.
A set of sealed official transcripts from all institutions of higher learning (Bachelors degree and onwards) which the applicant has attended must be sent to the address below. Minor transcripts for course-work that has no relevance to the degree can be omitted.
(Not required if the applicant has a PhD, JD, DDS, MD, or related degree from an accredited U.S. school or if applying to the Certificate Program.
International applicants with an MD from a non-US school may be eligible to waive this requirement. Contact the program for details.)
Have official GRE scores reported to the University of Washington (institution code 4854).
Useful GRE prep and math refresher courses are available through the UW Women's Center.
Three recent letters of recommendation are required. Recommendations should be requested of individuals who are familiar with your academic goals and your potential for achieving those goals. They should mention how long recommenders have known you and in what context.
Applicants will be asked to designate recommenders in the online application. The online application will then send specific instructions to each recommender. Applicants will be able to return to the online application to check whether the application system has received the recommendations.
Applicants are expected to be in contact with each recommender to make sure that the recommendations are sent to the online application system by the due dates.
If your referral has trouble accessing criteria for the recommendation on the Graduate School website, you are welcome to use the ExDP recommendation form which your referral can send directly to ExD.

A current resume or CV should be submitted with the application.
The Extended Degree Program requires all applicants to take a Quantitative Skills Test to assess their level of preparation for the quantitative portions of the program. Instructions for how to take The Quantitative Skills Test are available inside the online application. Use the sample questions below to prepare for the test.
Quantitative Skills Preparation:
Experience with courses taught during the first summer indicates that a review of college-level basic statistics and/or algebra is often essential for successful completion of the program. We strongly suggest "brushing up" on quantitative skills before attending the first summer session.
- Consider taking UW courses such as MATH 120, Precalculus or STAT 220, Basic Statistics.
- Similar courses are offered by local community colleges and distance format is often available.
- For basic algebra review consider books such as "Algebra for Dummies" or "Forgotten Algebra", both available at bookstores.
