Newly-admitted MPH Program and Certificate Students

Congratulations on being admitted to the EXDP program.

Required Forms:  Please begin to fill-out the required forms listed below and then begin reading through the information linked from the yellow Important Steps box to help you get started with the program.

The University of Washington makes a number of requirements of all students as well as a few specific requirements for health sciences students who may eventually work with the public during their project or practicum.

Please review each item below and return the completed forms directly to the EXDP program office as soon as possible.
Student Interest Survey

Please fill out this short survey of your interests and experience. The information will be distributed during the summer seminars to help the faculty and your fellow students get to know you.

Employer Verification of Intent Form

Designed to help notify employers about the challenges and potential benefits involved in their employees pursuing education while continuing to work.

Washington State Patrol Criminal History Background Check

State law requires this background check for all students who will participate in a practicum.

Proof of Measles Immunization

All students in the Health Sciences are required to provide proof of immunizations to the University Student Health Center.

Waivers

Some university requirements may be waived under certain circumstances.

Insurance Fee Waiver

Blood-Borne Pathogen (BBP) and HIPAA training
If you have previously completed training, please fill out the following document with either a supervisor's signature verification or attach a copy of certificate of completion. Send all documents to ExDP via fax, mail, or scanned signed copy via email. Electronic signatures are not accepted.

If you have not completed training for either of these, training will be provided for you by ExDP. That training occurs online between the Summer session and October seminar. Please contact the Student Services Coordinator, Cindy Moore (crmoore@u.washington.edu), to set up training. You will be directed to a website to complete training (free if done within 30 days of set-up--thereafter, a processing fee will be incurred). Once you have successfully completed training, send a copy of the completion certificate to the ExDP offices or email a copy.
