UW Insurance Enrollment Guidelines
Please read before enrolling:
- If your program crosses the August 15, 2009, policy renewal date, you will need to enroll in both the 2008 and 2009 policies to ensure coverage.
- You cannot pay for this insurance plan through MyUW, nor will the premium be charged to your UW student account or be automatically paid by financial aid. If you intend to use financial aid to reimburse yourself for the costs of the insurance you must submit an IPE Program Budget Sheet and Financial Aid Revision Request to the Financial Aid Office before your departure. Your financial aid will be disbursed on the first day of the applicable UW quarter(s) of enrollment.
- The UW Study Abroad Health Insurance plan is distinct from the UW Student Accident and Sickness Insurance Plan. Enrolling in the UW Seattle, Bothell or Tacoma student insurance plan for the same time period will not automatically enroll you in this plan.
- If you will receive a graduate appointment eligible for the Graduate Appointee Insurance Plan (GAIP) for your study or research abroad do not purchase this insurance. Coverage is already provided under the GAIP plan.
Enrollment Instructions:
To purchase the plan with a future effective date OR for one month only:
- Do not enroll online.
- Download and complete the enrollment form available via: https://www.uhcsr.com/washington
- Mail the form with your premium payment (check, money order, or credit card information) to the address in the Payment Instructions section of the form.
- Coverage will be effective on the requested date as long as the completed enrollment form and payment are received on or prior to that date. Otherwise, coverage will be effective the date the completed enrollment form and payment are received.
To purchase the plan with an immediate effective date:
- Enroll online: https://www.uhcsr.com/washington
- You must purchase a minimum of two months of coverage to be eligible to enroll online.
- Coverage will be effective the date online enrollment is completed (or the policy effective date, whichever is later).
