Licenses & Accreditation

All testing locations within the Department of Laboratory Medicine, UW Medicine are subject to the Clinical Laboratory Improvement Amendments of 1988 (CLIA-88). Washington State's clinical laboratory licensure program has been judged by the Federal Health and Human Services Centers for Medicare and Medicaid Services (CMS) as equivalent to CLIA and was granted an exemption from the federal Clinical Laboratory Improvement Amendments of 1988 (CLIA). Our CLIA number is maintained by the Washington State Department of Health as part of the Medical Test Site licensure program.

Our laboratories are located in six facilites in the University community, all of those locations have individual licenses and accreditations. Please refer to the list below for each locations' licenses, image of certification and testing services.

Laboratory Medicine has never experienced a limitation, suspension, probation or voluntary relinquishing of a license, certification or accreditation.
