Lab Testing Restrictions from New York State

The State of New York has placed restrictions on the performance of lab work for all specimens originating from within that state. Under New York State Law, Title V, laboratories must be licensed, or have been issued an individual test request approval, by the New York State Department of Health prior to performing testing on samples originating from New York State.

Our laboratory has NOT been certified by the New York State Department of Health. To enable us to perform testing on a specimen from that state, you must submit a “Non-Permitted Laboratory Test Request Approval Form” to the New York State Health Department and receive approval for the specific testing that you intend to send to our lab.

Once approval has been granted, a copy of the Approval Form should accompany the specimen and requisition sheet when they are shipped to us.

An Approval Form must be submitted to the New York Health Dept. each time that a specimen is sent to us for testing, and should accompany the specimen when it is shipped to us. We can also receive this document by FAX or mail.
