Application


Congratulations, you are ready to apply to the UW LEND Training Program.

Application Steps:
     
Step 1   Contact the UW LEND Training Director or Discipline Leader for your clinical discipline to discuss your interest in applying to the UW LEND program, and to obtain any specific application instruction.
     
Step 2  

Download and complete the UW LEND Application below - Please read instructions carefully to ensure your application will be saved correctly and readable when opened on other computer systems.

PDF Application
1. Save pdf file to your computer
2. Open pdf on your computer, fill out form and save the file with your last name as part of the filename.
3. MAC users: Be sure to use Acrobat Reader software to fill out your application! If you use Apple Preview your application data will NOT be readable on other computer systems. If you don't have Adobe Acrobat Reader software and don't want to install it you should fill out the Microsoft Word version at the bottom of Step 2.

4. Double check your information was saved by reopening file.
5. After verifying information is saved email as file attachment to Sally Stuart, PhD, LICSW (Note: Date field needs mm/dd/yyyy format)

MAC users or those who prefer to use Microsoft Word:

Step 3   Write a personal statement describing what you hope to gain from participating in an interdisciplinary program focused on leadership development relative to serving children with disabilities and their families.
     
Step 4   Secure two letters of reference from individuals familiar with your academic and/or professional clinical performance. Family members applying to the program may secure letters of reference from individuals who can speak to their experience with children with neurodevelopmental disabilities or related disabilities and their families, and can speak to their leadership potential.
     
Step 5   Acquire non-official copies of academic transcripts.
     
Step 6   Submit all materials to your designated UW LEND Faculty Mentor. Materials do not need to be submitted all at the same time.
