Registration Steps


UW LEND Trainees must take several steps to complete registration for the UW LEND Program. The registration steps are very important and must be completed within the first 30 days of starting the UW LEND training program. These steps assure that you have met University of Washington Medical Center and UW LEND compliance standards for participating in direct patient care and clinical service.

Step 1   All UW LEND Trainees must complete this step.
   
  • UW LEND Orientation
    Attend and participate in the UW LEND Orientation.

  • Trainee Registration
    Complete the UCEDD Trainee Registration Form and submit online.

  • Trainee Profile
    Complete the UW LEND Trainee Profile Worksheet and email or return a hardcopy to Nancy Saunders, rm CD 422.

  • Trainee Photograph
    Make an appointment with Nancy Saunders (206.221.6525) to have your photograph taken. You photo will be added to your UW LEND Trainee Profile.

  • Release of Information
    Complete UW-CHDD Release of Information Form and email or return a hardcopy to Nancy Saunders, rm CD 422.

  • Personal Contact
    Complete the Personal Contact Form and email or return a hardcopy to Nancy Saunders, rm CD 422. This information is used to locate you in the building.

  • UW NetID and Email
    Create a UW NetID and open a UW email account. Even if you do not plan to use a UW email account as your primary email address, please still create one. You will need a UW NetID for several electronic activities during your UW LEND training.

  • UWMC ID Badge
    All trainees must wear a UWMC ID badge while engaged in CHDD clinic activity.

    1) Email Gretchen Glass your UW Student ID# , UW Employee ID#, or social security number. Gretchen will contact UWMC Security on your behalf to start the process for obtaining a UWMC ID badge.

    2) You may obtain your UWMC ID Badge within 24 hours of Gretchen’s email to UWMC Security by visiting the UWMC Security Office in the UWMC room NW014.
    Drop in hours: M–TH 8am–4:30pm, F 7:30pm–4:30pm. (206.598.4907)
    **Bring piece of photo ID (driver’s license)**
   
     
Step 2   Part A: Will you be participating in direct patient care in CHDD Clinics?
If yes, compete the following:
(If no, go to Step 3)
   
  • TB Test
    You must provide written proof of current TB test or complete a TB test. You may obtain a TB test at UWMC Campus Health Clinic room NN256A. This is located across from Emergency on the 2nd floor.
    Drop in clinic hours: 7:30am – 12 noon and 1:00pm-4:00pm. (206.598.4848)
    **Return TB clearance form to Nancy Saunders, rm CD 422.**

  • Integrity at Work Brochure
    This brochure is included in your UW LEND Orientation packet, or you may obtain a copy from Nancy Saunders. Read the material, sign/date the final page, tear out that last page, and return this signed/dated page to Nancy Saunders, rm CD 422.

  • Data Security Form
    Read the material, print, complete/sign/date the Data Security Form PDF document and return to Nancy Saunders, rm CD 422.

Part B: Complete the following online modules.

  • UWMC 2008 Education Fair and Compliance Topics http://odt.uwmc.washington.edu/education/orientation/compliance.html
    Complete the following training modules. Print the 1 page confirmation of completion for each module and return the five pages to Nancy Saunders. (Hint: Print off copy of quiz and complete with pencil while viewing the topic content, use your hardcopy to fill-in answers on the electronic quiz.)
    • Cultural Diversity
    • Workplace Safety
    • Organizational Specific Topics
    • Compliance & Ethics Training
    • Clinical Topics

  • HIPAA and Corporate Fraud Awareness
    These are two separate online modules. If you have not already completed both through the University of Washington, contact Nancy Saunders. She will request a unique login/password for you. This takes approximately one week to obtain. Once you have the login/password, visit the website included in the instructions which are attached to the login/password document and complete both modules. Print the confirmation page/certificate page for each and return this to Nancy Saunders, rm CD 422. This must be done within 30 days of starting the UW LEND Training Program.
   
     
Step 3   Will you need access to UWMC electronic patient records to read reports? If yes, complete the following: (If no, go to Step 4)
   
   
     
Step 4  

Congratulations! You have completed the registrations steps that apply to you. Now, complete the following final activities:

  1. Print the Registration Checklist PDF document, check off completed steps, sign and date

  2. Gather all forms as requested in the registrations steps above

  3. Return all appropriate documents to:
