- UW LEND Orientation
Attend and participate in the UW LEND Orientation.
- Trainee Registration
Complete the UCEDD Trainee Registration Form and submit online.
- Trainee Profile
Complete the UW LEND Trainee Profile Worksheet and email or return a hardcopy to
Nancy Saunders, rm CD 422.
- Trainee Photograph
Make an appointment with Nancy Saunders (206.221.6525) to have your photograph taken. Your photo will be added to your UW LEND Trainee Profile.
- Release of Information
Complete UW-CHDD Release of Information Form and email or return a hardcopy to
Nancy Saunders, rm CD 422.
- Personal Contact
Complete the Personal Contact Form and email or return a hardcopy to Nancy Saunders, rm CD 422. This information is confidential for emergency contact and for UW LEND tracking.
- UW NetID and Email
Create a UW NetID and open an email account. You will need a UW NetID to connect to UW services, access your ePortfolio, and other electronic activities during your UW LEND training. If you are not eligible to create a UW NetID, please contact Nancy Saunders.
- UWMC ID Badge or Approved Nametag
All trainees must wear either a UWMC ID badge or other approved nametag while engaged in CHDD clinic activities.
- Email Gretchen Glass your UW Student ID# , UW Employee ID#, or social security number. Gretchen will contact UWMC Security on your behalf to start the process for obtaining a UWMC ID badge.
- You may obtain your UWMC ID Badge within 24 hours of Gretchen’s email. The UWMC Security Office is located at UWMC room BB120 (Ph: 206.598.4907).

These requirements must be completed within the first 30 days of starting the UW LEND training program. These requirements assure that you have met University of Washington Medical Center and UW LEND compliance standards for participating in direct patient care and clinical service. Return documentation indicating completion of the following to to Nancy Saunders, rm CD 422.
- TB Test
You must provide written proof of current TB test or complete a TB test each calendar year. You may obtain a TB test at UWMC Campus Health Clinic room NN256A. This is located across from Emergency on the 2nd floor. Drop in clinic hours: 7:30am – 12 noon and 1:00pm-4:00pm. (206.598.4848)
- Integrity at Work Brochure
Obtain a copy from Nancy Saunders. Read the material, sign/date and tear off the final page.
- Data Security Form
Read the material, print, complete/sign/date the Data Security Form.
- UWMC Annual Education Competencies
Complete the UWMC Annual Competences. Follow these instructions:

3. Read each of the competencies, complete and print the quiz.
- HIPAA
Contact Nancy Saunders for a unique login/password. This takes approximately one to two weeks to obtain. Once you have the login/password, visit the website included in the instructions which are attached to the login/password document and complete both modules. Print the confirmation/certificate page. This must be done within 30 days of starting the UW LEND Training Program.
