MHA Programs Application Instructions and Forms

Application Deadlines

Applications are accepted for Autumn Quarter only.
Different application deadlines apply for the MHA and Executive MHA programs, as well as for international applicants.

For US applicants:
Preference for admission review will be given to applications completed by the relevant MHA or Executive MHA deadline posted above. Applications completed after the relevant deadline will only be considered on a space-available basis. If interested in late application, please contact the Program Office to determine if we are still accepting applications.

For International applicants:
International applicants must meet the application requirements by the relevant deadline(s) posted above. For the MHA Program, different deadlines apply to the materials required by the UW Graduate School (www.grad.washington.edu/admissions/adminfo.html#International) and the materials required by the MHA Program. Late international applications will not be reviewed.

NOTE: Since the Executive MHA is a part-time, fee-based, program, applicants holding or expecting to receive an F-1 visa are not eligible to apply.

Admission Decisions:
MHA applicants can expect to receive notice on the status of their applications by the end of February. Executive MHA Applicants can expect to receive notice on the status of their applications by the end of May. Late applicants can expect to receive notice within two to four weeks of submission (depends on timing of submission).

Application Process

The MHA Program has collaborated with the UW Office of Graduate Admissions in the Graduate School to provide an online application process. The process includes application status tracking for both applicants and programs. All application steps or materials must be submitted through the online application unless otherwise noted. Please review the information below before proceeding.

NOTE: The MHA Program will not review your application until all items in the online application are marked as "completed."


MHA Concurrent Degree Applicants:

The application and admission review processes for the MHA and concurrent degrees (MBA, MD, and MPA) are separate. Applicants to concurrent degree programs must complete both the MHA and relevant concurrent degree applications. See the information at http://depts.washington.edu/mhap/concurrent regarding websites and contact information for each of the concurrent degree programs. In any case, be sure to indicate on the Supplemental Program Application form the concurrent degree program to which you are applying.

NOTE: If you are currently enrolled in the MBA, MD or MPA program and now wish to apply to the MHA, please contact the MHA Program Office before proceeding.


Required Application Materials:

Graduate School Applicant Profile:
As you begin the online application process, you will create an Applicant Profile containing basic applicant data, such as your name, contact information, and geographical information. Based on the information entered in your Applicant Profile, the system will identify your application type (i.e., US citizen or international, degree seeking or graduate non-matriculated, etc.) and present you with the relevant checklist of application requirements and step-by-step instructions. You will build upon your application by submitting the information or materials required to fulfill this checklist.

NOTE: After creating your Applicant Profile, you will be asked to select the degree program to which you are applying.

  • For the full-time MHA program select:
    Health Services Administration-In Residence (Day) Program (MHA )
  • For the Executive MHA program select:
    Health Services Administration-Executive Program (MHA )

Résumé/Curriculum Vitae (CV):
The résumé or curriculum vitae should include a complete history of your academic experience, professional/work experience, volunteer work, membership in professional organizations and/or licenses held, honors received (scholarships, awards, etc.), and publications (if any). Where appropriate, items listed should be interpreted in terms of their relevance to a health services administration career, and include beginning and end dates.

Recommendations:
Three recommendations are required. A specific form is provided to the recommender through the online application process. Recommendations should be requested of individuals who have knowledge of your academic performance and/or performance in a work setting and who can evaluate your professional abilities and potential for and commitment to a career in health services management. Recommendations from relevant work experience (vs. academic experience) are preferred. If applying to the Executive MHA, at least two recommendations should be from professional/work references. Personal references are not appropriate.

NOTE: The MHA Program requests that all applicants instruct recommenders to submit the form through the online process. If you or the recommender has any concerns regarding this, please contact the Program Office. In either case, we advise applicants to discuss their application with recommenders prior to designating them in the Applicant Profile.

In order to better understand the skills, attributes, and experience applicants might bring to the MHA Program, we require each applicant to complete a self-assessment derived from a select set of professional and leadership competencies defined by the National Center for Healthcare Leadership (NCHL) Competency Model.

The MHA Program requires an additional application form to supplement the information required by the Office of Graduate Admissions.

Applicants are asked to provide responses to four statements or questions which are outlined in the form linked above.

Official Transcripts:
Applicants must submit to the MHA Program Office one official (signed and sealed) transcript for each college/university attended. If you have earned a bachelor's or higher level degree from an institution outside the US or its territories, you must send an additional official transcript to the Graduate School, as specified in the Graduate School's application instructions.

Test Scores: (GRE / GMAT / MCAT)
Test scores are not required from individuals who have earned a master's degree within the last 10 years or a doctoral level degree (e.g., MD, PhD, PharmD, DDS, JD), from a US accredited institution. All other applicants must take the GRE* and have the scores reported to the Program (use Institution Code R4854). Scores must not be more than five years old from the application deadline. Applicants should plan to take the exam in time for scores to arrive by the application deadline. Call 1-800-GRE-CALL or visit the GRE web site (www.gre.org) for information. Applications for these exams can be obtained from any university or from:

* GMAT or MCAT scores are also acceptable. The GMAT is required for admission to the MHA/MBA Program and scores should be reported by the testing center to the Graduate School (for the MHA use Program Code 459-54-54; for the Executive MHA use Program Code 459-54-46). The MCAT is required for admission to the MHA/MD Program and scores may be submitted by photocopy to the MHA Program Office. Scores must not be more than five years old.

TOEFL Scores:
Applicants, including international applicants, permanent residents, and immigrants, who are not citizens of the United States, whose native language is other than English, and who have not received the equivalent of a bachelor's or higher level degree from an accredited US institution, must submit a valid official score from the Test of English as a Foreign Language-TOEFL. Only score reports less than two years old from the test date to the application deadline will be accepted. The minimum test score requirement is 237 (computer), 580 (written), or 45 (iBT-reading, listening, and writing). The University of Washington's Institution Code for score reporting is R4854.

No waivers of the English competency requirement may be given. For additional information, see the Graduate School website at http://www.grad.washington.edu/admissions/adminfo.html.

Application Fee:

A $50 application (processing) fee is required by the Graduate School for each application submitted.
