Project Application Form

All investigators must have a project Application form (PAF) approved by the MR Lab director prior to scheduling any MR scanning sessions for a new study. This applies to both funded and pilot studies. The DISC Operations Committee” reviews PAFs and provides recommendations to the director. The committee meets monthly (typically on the 3rd Friday). Your study can be reviewed if a complete “Project Application Form” (PAF) has been submitted. PAFs that are incomplete are never reviewed. To ensure a timely review it is a good idea to submit this form at least one week in advance. This early submission will allow for an initial administrative review. Every effort will be made to coordinate with you should minor deficiencies exist.

You will also need to include an approved consent form, an approved Human Subjects application, and an approved Animal Medicine Committee application (if applicable).

So, how does one complete a PAF? You might want to start by looking at the form to get an idea of what will be required. Some of the items in this checklist (and most of the information in the PAF itself) will be easy to provide. However, some parts of the required information require time, effort, and technical knowledge. So to be efficient in this process you need to get these items (J through R) under control soon as possible.
