Project Application Form

All investigators must have a project Application form (PAF) approved by the MR Lab director prior to scheduling any MR scanning sessions for a new study. This applies to both funded and pilot studies. The DISC Operations Committee” reviews PAFs and provides recommendations to the director. The committee meets monthly (typically on the 3rd Friday). Your study can be reviewed if a complete “Project Application Form” (PAF) has been submitted. PAFs that are incomplete are never reviewed. To ensure a timely review it is a good idea to submit this form at least one week in advance. This early submission will allow for an initial administrative review. Every effort will be made to coordinate with you should minor deficiencies exist.

You will also need to include an approved consent form, an approved Human Subjects application, and an approved Animal Medicine Committee application (if applicable).

    A CHECKLIST FOR A COMPLETETED PAF:
  • Completed Project Application Form
  • Accurate budget information or estimated hours for pilot study
  • Project description
  • Approved Human Subjects Application with approval # (if applicable)
  • Copy of approved consent form
  • Approved Animal Medicine Committee application with approval # (if applicable)
  • Protocol to be used in the study
  • PI signature

So, how does one complete a PAF? You might want to start by looking at the form to get an idea of what will be required. Some of the items in this checklist (and most of the information in the PAF itself) will be easy to provide. However, some parts of the required information require time, effort, and technical knowledge. So to be efficient in this process you need to get these items (J through R) under control soon as possible.

  • Request an MR Consult:
    • Why: All new investigators (and most seasoned veterans) can benefit from an initial contact with the director and/or his technical staff. This meeting will introduce your project and give us a chance to help you with PAF information that is not immediately obvious to you. Specifically this meeting can provide you with information about:
      • The imaging protocol (item Q)
      • Estimates of needed lab time (R)
      • Procedures to obtain approval for bringing equipment into the MR facility (P)
      • Possible use of the Center blanket consent form (J)
    • How: To request an MR consult please visit the DISC support center and issue a new ticket. In the Subject field of the resulting email interface please include the text Request initial MR Consult”. Then in the body of the email, provide us with text that tells us about your study.
  • Human / Animal subjects approval: Depending upon the study’s subject pool (Human / Animal) you will also need to include an approved consent form, an approved Human Subjects application, and an approved Animal Medicine Committee application (if applicable).
