Project 1: CKD- Targeted Analysis of the Jackson Heart Study (PI: Young)

This Jackson Heart Study (JHS) CKD Project will first develop a highly-functioning diverse CKD working group composed of national experts in health disparities, CKD epidemiology research, and CKD biostatistics. The working group will refine a research agenda to evaluate novel and traditional risk factors for CKD and the interrelation of CKD with CVD among African Americans enrolled in JHS.

Project 2: Clinical Program: Veterans Affairs Specialty Care Access Network – Extension for Community Healthcare Outcomes (VA SCAN-ECHO)(PI: Young)

This study will develop a dedicated team of specialist and other personnel for the SCAN-ECHO Core Teams, which will include specialists from the following disease areas: chronic kidney disease, Hepatitis C, Congestive Heart Failure, Advanced Lung Disease/COPD, and HIV. Leveraging the established VISN 20 telemedicine network infrastructure, we will partner with rural on-site primary care providers and hub VA specialists to develop SCAN-ECHO clinics with the VISN 20 that will provide Veterans and providers with access to specialty care in order to improve population-based management of complex chronic disease patients.
