Pre-arrival Paperwork Requirements

The application packet below must be completed AT LEAST THREE WEEKS PRIOR to beginning your clerkship.  Fax all items in the packet to the HR Office at Virginia Mason.

  • Common errors when submitting:
    • VM will do their own background check.  The 7-year address history IS needed.
    • When the pdf is printed by VM, electronic signatures and dates do not populate.  Please print the pdf and actively sign and date the forms before sending them back.

Course Information

Clerkship orientation: Orientation is at the UW at 8:00AM on the first day of your clerkship. The room number is on the Didactics page. Room information may change at the end of each rotation, so be sure to check the table the weekend before you begin.

After orientation: Report to Dr. Stoilova or Ms. Reisner at Virginia Mason Neuroscience Institute (address & map above).

Equipment to bring: White coat and name tag, reflex hammer, tuning fork (128Hz), stethoscope, (Ophthalmoscope recommended).
