Promoting Excellence in End-of-Life Care is a national program office of the Robert Wood Johnson Foundation. As part of their assistance to grantees who are addressing end-of-life care, they have developed extensive evaluation and measurement resources, with information and downloadable instruments. The website cautions that some instruments may require author permission to copy.

Part of the Center to Advance Palliative Care website, this article provides general guidance in assessing community needs for palliative care services. In addition, the site includes specific tools for assessing patient care as well as for evaluating organization and system-level processes.

Toolkit of Instruments to Measure End-of-Life Care - www.chcr.brown.edu/pcoc/toolkit.htm. Brown University’s Center for Gerontology and Health Care Research has assembled this compilation of instruments to assess end-of-life care. It offers detailed guidance in evaluation design and administering tools, and includes interviews for patients and family members.

Tools for Change - www.careofdying.org. This site features a compilation of tools provided by the coalition Supportive Care of the Dying. Instruments include the Organization Assessment Tools and Questionnaires, designed to provide information to organizations wishing to measure the quality of their services, and the Competency Standards and System Assessment Tool to help assess current standards for performance.
