 Course Objectives:

1. Describe the stages of advance care planning from early goals of care through end of life care.

2. Deploy a step-wise framework to elicit preferences for future health and respond to emotions that arise

    during advance care planning.

3. Narrow disparities in healthcare for patients with serious illness by building communication skills for working

    with patients with diverse racial, cultural and spiritual backgrounds and preferences especially as it relates

    to decision making and goals of care.

4. List available tools and resources to conduct advance care planning from patient-facing tools, to provider

     education, to system-wide strategies.

5. Assess different advance care planning needs for a range of populations including patients with cancer and

    perinatal serious illness.

6. Describe how current research in the field of palliative care is changing how healthcare is delivered.

7. Explain ethical challenges and strategies for using surrogates in health care decision making.

8. Describe the POLST form and the role the POLST plays in advance care planning.
