How to Apply
Applicants must apply to both
the University of Washington Graduate School and the Department of Pharmaceutics.
Applicants must meet the basic admission requirements of the University
of Washington Graduate School and the Department of Pharmaceutics graduate
program to which they are applying .
Domestic/International Applicants:
Please complete the application form online at https://www.grad.washington.edu/applForAdmiss/
Submission Process:
Applicants must submit
two separate packets of materials:
Send
First Packet to the Office of Graduate Admissions by Nov.
1 for international applicants; Jan 15
for domestic applicants
- Original copy
of UW Graduate School application form (signed/original signature
and dated)
- Online application
fee $45
- One copy of
official transcripts in a sealed envelope from each college you
have attended
- Official GRE
score report
- All foreign
students must take the TOEFL
and TSE
test and send in the official score report
- If currently
in the U.S., send photocopies of all I-20s (Certificate of Eligibility
Form) with the F-1 stamp or DS-2019 (Certificate of Eligibility)
with the J-1 stamp or any current visa status forms issued to you

- A copy of Graduate
School application (signed and dated)

- Statement of
personal goals describing your background, academic interests, and
career objectives
- A résumé
or curriculum vitae listing educational and employment history
- Three letters
of recommendation from persons
who are in a position to evaluate your potential for graduate school.
Your referees should return their recommendations directly to the
Department of Pharmaceutics in sealed envelopes with their signatures
over the seals
- Assistantship
and Fellowship application (this form can be downloaded, please
double click on it)
