indicated, a consult appointment with

Information
required from your office (we will not be able to provide accurate genetic counseling without the following):

medical
records confirming any family history of a heriditary disorder
-
prenatal
records, including all ultrasound reports to date
-
laboratory
results, including serum screen, and maternal blood type
-
insurance
authorization/referral form
What
your patient can expect during their visit with the genetic counselor:
-
a multi-generation
pedigree will be obtained
-
review
of prior pregnancy information
-
review
of their ongoing pregnancy, including history of exposure to medications,
illnesses and environmental hazards
-
this information
will be used to assess the patient's genetics risks and identify other
potential problems
-
the genetic
counselor will then explain these genetic risks and the available testing
options (integrated screening, carrier testing, US, amnio, CVS, 1st and 2nd trimester blood sampling) with the patient
-
the goal
of the counseling is to provide information and support during your patient's
decision-making process
What
your patient can expect if they have an ultrasound:

evaluation
of the uterine environment, including placenta and amniotic volume
-
detailed
anatomic survey of the fetus(es)
-
review
of findings with patient that day
What
your patient can expect if they see a perinatologist:
-
review
of all pertinent medical, obstetrical and family history
-
in-depth
discussion of appropriate evaluation and management options for specific
problems identified
Feedback:
-
The management
of the complex patient will be decided upon in conjunction with the referring
care provider. This often occurs via phone conversation between the
perinatologist and the provider. A consult letter will be mailed
to the provider.
-
Documentation
of the genetic counseling provided will also be sent to the provider.
-
Abnormal
test results will be called and faxed directly to the provider by
either the genetic counselor or the perinatologist and to the patient according to the referring providers preference.
-
Normal
test results are called out to the patient as soon as they are available.
A dictated results letter will be mailed to both the provider and patient to summarize the results.

Providers
can also access the information via the UWMC secure web site service known
as U-Link.
This
is an exciting and efficient method of obtaining feedback on all of your
referrals. All physicians are encouraged to apply for U-Link privileges.
