APPLY EARLY – do not wait until the last minute to request transcripts, letters of recommendation or to submit an application. The University of Washington School of Pharmacy Office of Academic and Student Programs will send a confirmation status e-mail to all applicants from whom we have received both the PharmCAS application and the UW School of Pharmacy Supplemental Application.

NOTE: The UW School of Pharmacy will not begin processing an application until we have received both the PharmCAS application and the UW Supplemental School of Pharmacy Application. Please note that after an application is submitted to PharmCAS, it will take 4-6 weeks for PharmCAS to verify transcripts, letters of recommendation, etc. and send us your application materials. After the School of Pharmacy has received your PharmCAS application, it will take a minimum additional 4 weeks to process and review your application.

PharmCAS Application Materials

• Deadline: January 5, 2010,11:59PM, Eastern Standard Time

Log in and complete PharmCAS Application Form

• Send appropriate fee to arrive at PharmCAS

• Official, sealed transcripts from ALL colleges or universities attended or from which credit was awarded. Transcripts are required even if coursework does not apply toward pharmacy prerequisites. For information on transcripts, please click here.

• Two (2) required recommendation forms and optional letters of recommendation. The University of Washington School of Pharmacy requires, and will accept, only two recommendations, one from an academic source and one from a professional source. Recommendations are NOT ACCEPTED from family members, personal health care providers, high school faculty/advisors, mentors or job shadow professionals.

  • Academic sources include: professors or teaching assistants who have directly interacted with the student in a classroom setting or official, university or college-affiliated academic advisors.
  • Professional sources include: employers or volunteer organization personnel with whom the applicant has directly worked.

• Foreign transcript evaluations. The UW School of Pharmacy will only accept evaluations by one of the following agencies:

• Request to have Official PCAT (Pharmacy College Admission Test) scores sent directly to PharmCAS. The last acceptable test date for 2009 application is in October 2008. For more information regarding the PCAT, please click here.

 

UW School of Pharmacy Supplemental Application Materials

Deadline: Postmark January 5, 2010 OR hand-deliver application to the School of Pharmacy Office of Academic and Student Programs, by the close of business, 5PM, Pacific Standard Time on January 5, 2010.

• Complete and download the UW School of Pharmacy Application Form, Essay and Checklist available on line. Applicants may also e-mail pharminf@u.washington.edu or call 206–685–2715 to request an application packet.

• Sealed, official foreign transcripts (not evaluations). Evaluations must be sent to PharmCAS.

• If “Permanent Resident” or "Refugee-Asylee" visa status – photocopy front and back of visa.

• A nonrefundable $45 application fee (make check or money order payable to the University of Washington). UW Student Fiscal Services converts all checks into electronic fund transfers. You will not receive your cancelled check because banking regulations require that the check be destroyed after conversion to electronic transfer.

• Official Advanced Placement or International Baccalaureate Test Score Sheet (only required if AP/IB scores are being use toward pre-requisite coursework). CLEP credits are not accepted.
