Clinical & Affiliate Faculty Guidelines & Process Information

Volunteer pharmacists are critical to the success of a modern school of pharmacy educational program. Without the selfless assistance of hundreds of pharmacists and other health care practioners, we could not produce quality graduates.

The School does not have many ways to say "thank you" for your time and energy. One thing we can do is appoint qualified volunteer preceptors to our Clinical and Affiliate Faculty. In addition to a faculty title such as Clinical Instructor, an appointment to the faculty will allow you access to our wonderful electronic health sciences library.

If you wish to investigate this process, please view or download the Appointment, Retention and Promotion Guidelines for Clinical and Affiliate Faculty.

Clinical & Affiliate Faculty Appointment Process

If you believe you qualify for appointment to our Clinical or Affiliate faculty (CAF), outlined in the above document, please have a voting faculty member from the Department of Pharmacy submit a nomination letter to arianag@u.washington or via US mail to:

UW School of Pharmacy
Attn: Ariana Ghasedi
BOX 357631
Seattle, WA 98195-7631

Once the nomination letter is received the nominee will be contacted to submit the following documentation:

Please note that pharmacy residents applying for Acting Instructor appointments will need to submit a nomination letter from a Department faculty member and a letter of support from the director of the residency program.

Department of Pharmacy faculty will vote on CAF nominees at the monthly faculty meetings in January, June and September of every year.  Please keep this voting schedule in mind when submitting nomination letters and supporting documentation.  Nominees will be notified via email of the vote outcome.

Retention & Promotion Process

To remain an active CAF member submission of a current Curriculum Vitae or Preceptor Information Update Form, which summarizes your service to the Department over the past year and other significant professional and scholarly accomplishments, is due no later than the end of November every year.

Information submitted will be used by the Clinical & Affiliate Faculty Retention & Promotion Committee during the annual retention review and for consideration of promotion.

Access to our Preceptor Information Update web site is through our Preceptor website or submit a current Curriculum Vitae to arianag@u.washington.edu or via US mail to:

UW School of Pharmacy
Attn: Ariana Ghasedi
BOX 357631
Seattle, WA 98195-7631

CAF Retention & Promotion Committee meets in late November/early December every year to determine reappointment and promotion.

Current CAF members will be notified via US mail if they have been reappointed and/or if they are being considered for promotion.
