Graduate School Application
Applicants who meet the program requirements must submit the University of Washington online Graduate School application. Applicants who apply through SOPHAS (Schools of Public Health Application Service) also must submit the UW online application.
A printed copy of the online application must be mailed to us along with your other application materials.
