Application Requirements and Forms
Degree Program
Applicants who meet the program requirements must submit a University of Washington Graduate School application AND an Institute for Public Heath Genetics departmental application.
The Graduate School application must be completed online. A printed copy of the Graduate School application must be mailed to us along with your other application materials. The Public Health Genetics departmental application cannot be completed online, and we regret that we cannot accept completed applications via email. The Public Health Genetics departmental application form should be downloaded, printed, completed, and mailed by regular mail.
