MONTHLY STATUS REPORTS

At the end of every month, a monthly status report should be submitted. It can be submitted by email (janessa@u.washington.edu) or by fax (+1 206-685-2034).

HOME VISIT TRACKING FORMS

If you are having trouble downloading the form or need the password, please email Janessa at janessa@u.washington.edu.

DIRECTIONS: You can download this form. Simply click the above link to open the form. Then save the file on your desktop. Now you can simply enter the information into the form, go online, and click submit.

You do not need to be online the whole time.

If you would like to submit multiple forms at one time:

1. Save multiple copies on your desktop (for example, save them as form1.pdf, form2.pdf, form3.pdf...). You can do as many as you want.
   
2. Enter information in each form (do not close them, leave them all open).
3. Next, go online.
4. While online, go to each form, and click the green submit button.
5. Close each file when you have finished (the information will not be saved!)
