Transition to Phenyl-Free 2

Our general strategy for transition from Phenyl-Free to Phenyl-Free 2 has been as follows:

We prepared a handout for the clinic visit that discussed the changes and the transition process. We are requesting that each family have a clinic appointment in which transition is discussed and a transition pattern, individual to each child, is developed. For example, we suggested that, for the youngest children, Phenyl-Free 2 replace Phenyl Free in 5 to 10 gram increments. We suggested 10 to 20 gram increments for school-age children and 40 gram increments for adolescents and adults. We also suggested that the transition process be completed in 6 weeks or longer.

In general, the process is proceeding very well. Several families are moving forward very slowly; most have moved fairly quickly and without problems. The toddler group (1 1/2 to 4 year olds) is moving through the process the slowest. This seems to be related to the developmental stage they are at. The kindergarten and grade 1 group is having few problems because, in general, they are able to understand the process of transition and are willing to do it. The are also willing to take the new product to school and receive support from friends. We are conducting a weekly telephone survey to keep tabs on the process.

The process of transition is important. The children need to know what is happening and, at some level, be in charge of how fast they move to the new product.

The transition process that the UW PKU Clinic uses is described in the following three files:

Family letter A letter that introduces the new formula to the families

Transition guidelines The protocol that the UW PKU Clinic followed. This information was reviewed at a clinic appointment and provided to families as a handout.

Transition worksheet A worksheet to help "map out" the transition process
