Data Request Form

This form helps you describe your data requests. Decisions as to which data are released are ultimately made by the PM Center steering committee. If your requests are approved we will use the information in the form to help prepare your data.

All of the data that you request will be archived on the PM Center server. The procedures for archiving datasets are described in section 9.2 of the data core's SOP. PM Center datasets are updated periodically. I will notify you when one of your datasets is modified so that a new analysis dataset can be created. If you are conducting a long-term analysis, expect that your dataset will change before your analysis is completed. For this reason, we strongly advise that you are able to easily reproduce any changes you make to a dataset we provide.

Instructions:
Complete the following form describing your data request. Fields with * are required fields. If your data request is not covered by one of the check boxes, type your request in the "Other info" section.
