General Patient Information
If you call for an appointment for yourself, the program nurse will review with you the reasons you desire to be seen in the program.  If you have a current or prior care provider, he/she may refer you to be seen in our clinic.  Either way, having complete records available to us prior to your first visit is very helpful in making a complete evaluation.  The program nurse can assist you with this process.

Prior to calling for an appointment, you should prepare yourself to answer several questions: 

  • personal medical history
  • pregnancy history including dates, outcomes,and/or complications
  • expectations of care for this pregnancy
In order to enhance the care you receive, please feel free to download and fill in the general medical/obstetrical history  and bring it with you to your appointment. This form should not take more than thirty (30) minutes of your time to fill out.  You may also fax the forms back to 206-598-4694, Attention:  Prematurity Prevention Program

If an ultrasound has been been scheduled for you, you do NOT need to have a full bladder.  In fact, most likely you will be asked to empty your bladder before starting the procedure.

All information is confidential and will not be discussed with others, including family members, without your prior consent.
