(click here for Payment by Credit Card form). NOTE: Your registration will not be
confirmed unless payment is received or scholarship is awarded. Participants are
responsible for their own room and board.
Payment by credit card can be emailed, faxed, or mailed. If
paying by check, please request an invoice number from Christine Makela.
Checks cannot be accepted without a UW invoice number.

Cancellation & Refunds
Refunds, minus a $75 cancellation fee, will be issued up to ten working
days prior to the course. After that, tuition fees are forfeited.
Cancellations must be received in writing - by letter, fax or e-mail.
Scholarship Information
Scholarship recipients will be selected
by the Course Directors on the basis of financial need and relevance of
the course to their career in STD/HIV research. We expect competition for
these scholarships to be intense, so we recommend that prospective
participants seek external resources to attend the course whenever
possible. Interested scholarship applicants should include with their
application a statement documenting financial need, and a description (not
to exceed one page, double-spaced) of how the course will contribute to
their development as a researcher in the STD/HIV field.
University Credit
University of Washington academic credit is available for matriculated students
(3 credits). If university course credit is desired, please indicate this on
the application form. Students taking this course for credit hours must
complete the Principles Course application and be enrolled for summer quarter.
