7. Medical Licenses -- Renewal and Reimbursement
Procedures
Washington State Medical License: To renew your Washington State medical license you must send your payment directly to Olympia. In order to receive reimbursement, once you receive your renewed license, send a copy of it and the completed reimbursement form to UWP, Camapus Box 359110.
DEA License: DEA licenses can be renewed online. Follow the instructions listed on the renewal form you receive from DEA. If your primary place of practice is UWMC or Harborview, you are fee exempt. Follow the instructions listed in "Medical Staff DEA". If your primary place of practice is NOT UWMC or Harborview, please contact your appropriate institution for their process.
