We strongly encourage you to complete your application as early as possible.
The next quarter for application is Autumn 2007. Students are not accepted
for other quarters due to the sequencing of the coureses.
APPLICATION DEADLINE
January 15, 2007 for U.S. citizens and permanent
residents.
November 1, 2006 (recommended) for international applicants
due to the time necessary for processing application materials.
APPLICATION PROCEDURE
UW Graduate School Materials Include:
On-line application at http://www.grad.washington.edu/admissions/adminfo.html.
The cost is $45. Additional information regarding Graduate Admissions
requirements can be found at their web
site.
Official GRE scores of the general test (verbal, quantitative
and analytical): Request that the Educational
Testing Service (ETS). forward your official score to the University
of Washington (Institution Code 4854).
International applicants must send one copy of their
official transcript to the Office
of Graduate Admissions as well as to the QERM program office.
QERM Program Materials Include:
One copy of the UW
Application for Admission to the Graduate School. (Please
print a copy from the web application to include with your QERM materials.)
Original Transcript: Original transripts in sealed
envelopes including courses, grades and degrees from institution attended
after secondary school. Transcripts are required from all institutions
from which you earned a degree, attended at least one academic year,
or took courses relevant to QERM. Transcripts are not required from
institutions where you took only a few courses unrelated to QERM.
Photocopy of your GRE General
Test scores: ETS should forward the official copy to the
University of Washington (Institution code 4854). The exam must be taken
within 5 years of admission.
Three letters of recommendation, including the signed
Graduate Applicant Recommendation
Form (PDF): Recommendations should be from faculty or scientists
familiar with your academic achievements and/or research experience.
Include the letters in sealed envelopes (signed on the seal) with your
other application materials. If necessary, letters may be sent separately.
Applicants who have been out of school for some time may include recommendations
from employers, but all efforts should be made to include at least one
letter from academic faculty.
Curriculum Vitae or Resume including:
- Professional experience (fellowships, assistantships, teaching).
Include position, approximate dates, duties and name of supervisor.
- Honors awarded for scholarship (scholarships, honorary society memberships,
prizes, etc.).
- Research experience (description, name of advisor and duties).
- Relevant employment and professional experience (description and
dates).
- Special skills (computer programs, etc.).
- Publications in journals, abstracts or conference proceedings (including
submitted papers) if applicable.
- Present position or activities.
Goal Statement: This statement should be limited to
1-2 pages, single-spaced. Discuss your educational and professional
background, especially any research experience, reasons for seeking
admission, educational plans at the University of Washington, career
goals and how the proposed QERM degree would contribute to those goals.
Optional
Diversity Information: Consistent with our goal of enriching
the intellectual and social enrichment of the University community,
the University of Washington and QERM program actively seek and welcome
students from diverse backgrounds. If you wish to have factors such
as varied cultural experiences, or educational or socioeconimically
disadvantaged backgrounds included in the review of your application,
write a brief statement not to exceed one page. The statement should
address the ways those experiences have affected the developmwent of
your special interests, career plans, future goals, and how you may
have overcome various obstacles.
Without a diversity statement, ethnicity information, except membership
in a Native American tribe, may not be considered in admission decisions
according to the Washington State law.
