If you have a patient you would like to enroll in the Registry, complete a Screening Checklist and fax it us at one of our Registry offices. If your patient meets the eligibility criteria, we will send you a registration packet to complete.
A final determination of your patient's eligibility will be made only after an Advisory Board physician has reviewed a completed registration packet.
