Assessing the Effectiveness of Programs, Policies, or Practices that Affect Social Determinants of Health to Promote Health Equity and Reduce Health Disparities in Chronic Diseases

This notice of funding opportunity is intended to support rigorous evaluation of the effectiveness and cost-effectiveness of existing or imminently available programs, policies, or practices that address Social Determinants of Health (SDOH) factors and how changes in SDOH improve health disparities related to chronic disease outcomes. It is not intended to fund studies with only individual-level exposure and outcomes, simple pre-post designs without an appropriate comparison or support the initiation and delivery of new programs.

There are three components of this NOFO:

  • Component A: Research Studies to Assess the Impact of Programs, Policies, or Practices that Address SDOH on Chronic Disease-Related Health Disparities
  • Component B: SDOH Simulation Model Development
  • Component C: Coordinating Center

Application Instructions

Please submit:

  1. A one‐page letter of intent with a description of proposed aims and approach.
  2. If the final application requires a diversity statement or statement of broader impacts, please summarize your plans to address the specific requirements on an additional page.
  3. CV (not biosketch) of the PI
