Prescription Refills
If you have refills remaining on your prescription, please contact the UW Medicine-affiliated pharmacy that filled your original prescription and have the prescription number on the bottle ready to give. You may also request your prescription refill online at the Department of Pharmacy Services.

Referrals
Patients on managed care plans should be referred by a primary care provider. Self-referrals are not accepted.
To make a referral, please use our UW Medicine Referral Request (PDF). The completed form can be faxed to the clinic.
