1.  Regarding your work with the agents noted below.  If these agents are in powder form they must be weighed, reconstituted, and diluted in a chem fume hood or a ducted biosafety cabinet wearing compatible chem resistant gloves and other appropriate Personal Protective Equipment (PPE) including safety eyewear.

 

If these agents are in vial form requiring reconstitution, they must be reconstituted in a chem fume hood or a ducted biosafety cabinet wearing compatible chem resistant gloves and other appropriate PPE including safety eyewear.

 

As these agents are either: (a) have a Health Hazard rating of 3, (b) a reproductive hazard, (c) are noted as toxic, (d) may cause harm to an unborn child, (e) are sensitizers or may cause sensitization through the inhalation or dermal route, or (f) may cause heritable genetic effects, personnel working with any of these agents must read their Material Safety Data Sheet (MSDS) to address the requirement for hazard awareness training.

2.  Regarding your work with Cisplatin, I have attached EH&SŐ SOP for Anti-neoplastic agent use in the lab, for your implementation.

 

3.  Regarding your work with the agents listed below.  You must contact Katia Harb, Industrial Hygienist, at 616-3771 to develop or review your SOPs for work with these agents.  If these agents are in powder form they must be weighed, reconstituted, and diluted in a chem fume hood or a ducted biosafety cabinet wearing compatible chem resistant gloves and other appropriate Personal Protective Equipment (PPE) including safety eyewear.

 

If these agents are in vial form requiring reconstitution, they must be reconstituted in a chem fume hood or a ducted biosafety cabinet wearing compatible chem resistant gloves and other appropriate PPE including safety eyewear.
