Job description
Provides pharmaceutical care to patients. Participates as a member of the Pain Management Team and the Pharmacy and Therapeutics Committee. Processes new prescriptions and refills, ensuring accuracy and appropriateness. Performs utilization reviews and performance improvement activities. Provides education to patients, providers and clinic staff. Works with health care providers in both consultative and collaborative role. Plays a key role with clinical services by verifying the medical necessity of pharmaceuticals. Assists with administrative activities.

This site is eligible for the Indian Health Service Loan Repayment Program. This is a great opportunity for those candidates who have just graduated or finished a residency program or those with current student loans. The loan money awarded is in addition to the salary.

Details
Duties and Responsibilities:

• Assist in development and implementation of policies and procedures pertaining to collaborative drug therapy management and clinic pharmacy services
• Independently process new prescriptions and process and authorize refills
• Assess appropriateness of drug to disease state and/or indicators
• Perform chart reviews of refill requests to ensure compliance with drug therapy and medical follow-up. Order laboratory tests as indicated or as necessary
• Dispense medications to patients with verbal and/or written information
• Perform drug utilization reviews addressing drug usage patterns, adverse reactions, misuse and high cost
• Research new drugs and make recommendations to professional staff
• Educate health care staff about drugs
• Assist in maintaining drug formulary and inventory control
• Assist in precepting health care professional students and residents in training
• Handle in-person, written and telephone requests for information and advice
• Maintain accurate records in the RPMS Electronic Health Record
• Comply with established standards of pharmaceutical practices

Physical Demands:

• Requires visual and hearing acuity and manual dexterity to utilize computers and standard office and audiovisual equipment
• Requires ability to use a telephone
• Requires sufficient hearing and speech ability to communicate effectively
• Requires ability to wear personal protective equipment
• Requires ability to move or carry job-related objects and materials
• Requires ability to give clear directions, understand instructions, verbal, numeric and spatial aptitudes, perception of detail, finger and manual dexterity, analytical and problem solving ability, alertness, memory, concentration, reasonability, and judgment.

Working Conditions:
Work is performed in an outpatient, ambulatory care medical clinic with in-house pharmacy. Long periods of sitting, standing, reaching and bending may be required. Work environment requires exposure to chemicals, toxic substances, communicable diseases, bodily fluids, x-ray, weather hazards, human behavior, fumes and odors. Requires frequent computer use. Duties may require travel.

Requirements
Education:
Bachelors of Science in Pharmacy required. Doctorate of Pharmacy preferred.

Experience:
Years of clinical practice experience (at least 50% of time) in outpatient/ambulatory care:

1. BS without residency = 5 years
2. BS with residency = 3 years
3. BS with general and specialty residency = 0 years
4. PharmD without residency = 3 years
5. PharmD with residency = 0 years
6. PharmD graduate who completed clinical clerkships within the last six months considered
