Job description
The Clinical Pharmacist at Northwest Hospital performs within broad
parameters defined by general organizational regulations and procedures. Responsibilities
include processing medication orders while monitoring potential drug allergies
and interactions, individualizing medication regimens using sound principles,
participating in activities to promote the practice of clinical pharmacy,
performance improvement, and quality assurance programs, utilizing approved
clinical protocols (e.g. TPN, therapeutic drug monitoring for aminoglycosides
and vancomycin, renal dosing, outpatient Aranesp management for chronic kidney
disease, warfarin dosing and management, etc…), providing comprehensive
pharmaceutical services to improve drug usage and therapeutic outcomes including
advising physicians on issues concerning drug therapy, and participating in
patient care rounds with multidisciplinary teams to evaluate patient progress.
The pharmacist also serves as a mentor for pharmacy interns and students in
their practical experiences.

Requirements
· Graduate from an ACPE accredited college of Pharmacy
· Current pharmacy licensure in the state of Washington
