Registration and Scholarship Information

Registration fee per module: $650 (USD); Early-bird rate: $550 (USD).

Reduced academic and government registration fee per module: $500 (USD);
Early-bird rate: $400 (USD).

Early-bird deadline is Monday, 11 June 2012.

Registration fees cover tuition, course materials, coffee breaks and networking events. Meals, travel and lodging are not covered. No textbooks are required or supplied, but books recommended as background reading are listed in the module descriptions.

Refund Policy: A $200 processing fee will be deducted from refunds requested after Monday, 25 June 2012. No refunds will be processed after Monday, 2 July 2012.

Payment Method: Payment can be made with Visa and Mastercard credit cards online via a secure server. Mailed payments to the mailing address shown below can be made with a purchase order (U.S. companies and organizations only) or by check or money order in U.S. dollars drawn on a U.S. bank. Checks should be made payable to the University of Washington. Please email sismid@uw.edu for wire-transfer instructions.

SCHOLARSHIPS

There will be some registration-fee and travel scholarships available in 2012 for students and post-doctoral fellows. Students from underrepresented groups, such as minorities and women, are encouraged to apply.

Applications are due on or before Friday, 13 April 2012, and recipients will be notified by Friday, 30 April 2012.

There is no application form to complete; however, applications should:

  1. Register early for the modules and check the “Yes&lrquo; button to the question, “Are you applying for a Summer Institute scholarship?” so that payment will not be required at time of registration. You will receive an email confirming your registration and a copy of your Scholarship Application Cover Sheet (.html format);

  2. Submit the following four (4) required items:

    • a copy of your Scholarship Application Cover Sheet,
    • a one-page CV,
    • a letter, addressed to: Prof. M. Elizabeth Halloran, Summer Institute in Statistics and Modeling in Infectious Diseases, explaining your reason for wanting to attend the Institute and the modules you wish to atend, and
    • a letter from your advisor or supervisor

Applicants may apply to both the Summer Institute in Statistics and Modeling in Infectious Diseases and the Summer Institute in Statistical Genetics for scholarship support for particular modules. Applicants should submit separate applications to both Institutes, stating in both applications why you are interested in that mix of modules.

Colleagues and students based in Africa might consider applying to the “Clinic onĀ  the Meaningful Modeling of Epidemiologic Data” with a focus on infectious disease dynamics in Capetown, South Africa 27 May through 8 June 2012.

Due to University policies, we are unable to grant travel scholarships to applicants residing outside of the United States. However, we can grant registration-fee (tuition) scholarships to applicants residing outside of the United States.
