You will find several questionnaires throughout this site. Your input as a caregiver and/or professional is essential in expanding our knowledge on this topic. The information that you provide when you complete the questionnaires will serve as data for research on the occurrence of social communication problems in school-age children. Each questionnaire contains a variety of questions for you to answer. We would like for you to complete each of the questionnaires, but you can always skip a question. If you visit our Web site on more than one occasion, you do not need to complete the questionnaires again. When completing the questionnaires, you will never be asked to provide identifying information about yourself or children; your identity is always completely anonymous. Once you have completed a questionnaire, you will be asked to "SUBMIT" your answers. Pressing this button will submit your answers, at which time they will become anonymous data for our research. We will be analyzing these data at a later time to determine more about the nature of social communication problems in school-age children. If you do not wish to have your answers be a part of this research, please do not "Submit" the answers to the questionnaires.
