Below are links to two questionnaires. The first questionnaire is designed for Professionals; the second for Professionals and Caregivers.

The first questionnaire will assist US

in gathering information about the prevalence of occurrence of social communication problems in the schools, including social communication behaviors, language abilities, social-cognition, and processing. The data from this questionnaire will be valuable in helping to develop policy for serving this population.

The second questionnaire is designed to assist YOU

in planning the assessment and treatment of a specific child with social communication problems. This questionnaire will yield a profile that will provide some directions for supporting your assessment and treatment decisions.

As you complete these questionnaires, you might find it helpful to refer to the definitions we presented earlier in the social communication model. Additionally, links have been provided throughout the form to provide definitions of main concepts.
