     for Caregivers and Professionals

We would like to provide you with assistance in profiling the strengths and weaknesses of a specific child with social communication problems using our model. To do so, we will be asking you specific questions about a specific child that you have in mind. These questions correspond to areas that comprise our social communication model. If you are visiting this site for the first time and have not read about the model, you may wish to do so; otherwise feel free to proceed. Please think carefully about "your child" and answer the questions in relationship to him/her. At the end, your answers will be organized in a "profile" that will provide direction about assessment for your child. You are welcome to complete this questionnaire as many time as you wish for different children; you will receive a profile for each child. For each child, just complete the full questionnaire, including the identifying information.

Note: Please do not at any time answer a question using personal identifying information (i.e., your name or the name of the child you are profiling).
