WHEN WILL THIS PROGRAM TAKE PLACE?

Pretreatment Assessment - Spring Quarter 2009

Two (2) sessions of approximately 2 hours each, schedule to be arranged.

• Assessments will take place between April 6 - June 10, 2009
• Sessions will be 2 hour blocks between 11:00 and 3:00 pm

Treatment - Summer Quarter 2009

4 sessions, 2 days a week (Two 50-minute individual and Two 90-minute group sessions), schedules to be arranged

• Treatments will be either Monday/Wednesdays or Tuesday/Thursdays
• Treatment will be accross 7 weeks between June 29 - August 14, 200

TOTAL COST AND PAYMENT INFORMATION

• Total cost of the program is $350. This includes the Spring assessment, and the Summer group and individual sessions
  
• Payment schedule is as follows:
  
  • $100 deposit due at the 1st day of evaluations.
    
  • $250 is due on the 1st day of Summer Quarter Monday, June 29th, 2009
    
• Please note that for this special program a reduced fee is not available.
