Recommendation Form
Three recommendations are required. More are
not necessarily helpful.
The evaluator should use the form provided below although a letter
is also acceptable. The recommendation should be returned to you
for inclusion in the Document Packet which you send to the department. Applicants must ensure that
we receive three recommendations before their application can be considered.
Fill out the top section of the recommendation
form before giving it to an evaluator. Please
make sure each evaluator has sealed the completed
form or letter in a business-size envelope and
signed the seal in ink prior to returning it
to you for inclusion in the Document Packet.
(If the evaluator prefers, the recommendation
can be mailed directly to us by the evaluator.)
Applicants should obtain recommendations from
persons in speech and hearing sciences or related
academic or work settings. The Department will
accept recommendations from evaluators in related
disciplines such as psychology, social work,
linguistics, education, nursing, physical/occupational
therapy or medicine. |
