How to Apply

The deadline for Autumn quarter admission is February 15

Please complete the following steps:

1. You must have identified a mentor with whom you wish to study. The first step in the application process is to contact your proposed mentor directly. Please do not submit an application unless you have corresponded with this mentor.

2. Submit the UW Graduate School Application (online application) and pay the application fee.

3. Submit the Speech and Hearing Sciences Application (PDF), including the Goals Statement described in the application form.

4. Submit sealed copies of transcripts from every university or college you have attended. If you attended a school which will not release sealed records to students, you may have these sent to the Department under separate cover.

5. Submit three Recommendation Forms (PDF). These must be sealed and signed across the back of the envelope by the evaluator prior to including in your application packet. They may also be sent under separate cover by the evaluator.          

6. Submit Graduate Record Examination Scores from the Educational Testing Service for a test taken within the last five years.

7. Submit a photocopy of your master's thesis, if you have one. If you do not, a sample of your professional writing would be helpful for faculty to review.
