Applications for Full Review
Research falling into categories considered to be of more than minimal risk to subjects requires review by the Institutional Review Board (IRB), which convenes four times a month at the UW Human Subjects Division (HSD). If your study requires full review, complete form UW13-11, which can be downloaded from http://www.washington.edu/research/hsd/form.php?id=32.
Associate Dean David Takeuchi is available to review a draft of your application before the application is reviewed by the IRB. Additionally, check out the Human Subject Division's page on common problems encountered in the review process at http://www.washington.edu/research/hsd/faq.php. When your application is final, including your faculty advisor's signature if you are a student, submit the application plus materials, including, if appropriate, a consent form, approach scripts, questionnaire, and letters of cooperation to David Takeuchi for review and signature. Your application and materials will be returned to you for copying and delivery to the UW Human Subjects Division at 3945 15th Avenue N.E.
Following is an example of a research study needing full review:
A research project will conduct face-to-face interviews with their subjects, who are young adults, ages 16-23, about their health habits. After the initial interview, subjects will be contacted once a week by a member of the research staff to answer questions about that week's health-related activities.
