Forms - Instructions
Below is a link to all the forms you'll need to use during your practicum experiences.
We've organized them by program--BASW, MSW Foundation Year, and MSW Concentration Year. Some forms are used by all students regardless of their program (like the Risk Release Form). These forms are available in the following two electronic formats (you only need one version or the other, not both):
1. Adobe Acrobat PDF (This format is designed so you can
print the blank form and type in the information using your
typewriter or handprint clearly).
In PDF
format, these forms can only be viewed and printed out for your
handwritten or typewritten input. The PDF documents require
an Acrobat file viewer to view and print, such as Acrobat Reader
from Adobe Systems. If you do not already have the Adobe Acrobat
Reader installed, you can download and install the free Adobe
Acrobat Reader from Adobe's
web site (Please follow the instructions on the Adobe website
for installation of the software).
2. Microsoft Word
(This format is designed so you can "fill in the blanks"
online and print the completed form directly from your computer).
The Word versions are bigger files, and so may take longer
to download depending on your connection speed. You
must have Microsoft WORD 6.0 or higher in order to view
and fill in the forms.
In Word
format, these forms allow you to type your responses directly
into them when opened with Word (use the TAB key to move between
fields to type your responses). We do not yet have the
capacity to securely receive forms completed by you through
email attachments. Please DO NOT send your completed
forms to us electronically!! Please print out your
form, complete it, sign it, and either return it to the Practicum
Office by mail, or in person.
AND ONE MORE THING... You'll notice that there is no familiar cover sheet on these versions of the evaluation forms. That's because we ABSOLUTELY MUST HAVE a signed hard copy of that cover sheet in order to submit a grade for your practicum hours. If you download a file to complete on your computer, please complete the front, signature page that was distributed to the student mail files and attach it to your evaluation before sending it in.
