Depending on your home department (Biostatistics, Statistics, other), these directions might differ slightly. Please contact your statgen advisor, or Elizabeth Thompson or Stephanie Monks if you have questions about procedures in your department.

1. Fill out the certificate checklist available here
2. Have your certificate advisor sign the form
3. Provide the graduate student coordinator (``SGSC'' in Biostat) with the signed form so they can verify the form's accuracy.
4. The form should then be filed appropriately (in Biostat, the SGSC will put the form in your file)
5. (For Biostat) The SGSC will send a copy of the completed form toe the faculty coordinator for the interdiscplinary certificate (currently Elizabeth Thompson )
