Dear UW Parents,

Last month our newsletter focused on wellness, broadly speaking, and about helping your student make good choices as he or she begins the college experience. This month, I have invited my colleague DC Dugdale, the Director of Hall Health Center, to talk to you more specifically about healthcare and a few to-dos before the academic year begins.

As always, I look forward to hearing about your thoughts and concerns as you and your student begin this year's endeavors.

Sincerely yours,

Eric S. Godfrey
Vice President & Vice Provost for Student Life

Dear UW Parents and Guardians,

Congratulations on your student's enrollment at the University of Washington!

Hall Health Center is the UW's Student Health Service. Located on the Seattle campus, we are an outpatient clinic that provides health and medical care to UW students. We are committed to keeping your student healthy so that he or she can succeed academically and make the most of his or her student experience at UW.

All registered UW students are eligible for medical care at Hall Health. Many services are offered at no charge, and the Services and Activities Fee pays for one free medical office visit per quarter for medical concerns. This this excludes comprehensive physicals, specialty visits, and procedures. Please see our website for a detailed description of prepaid services available to currently enrolled UW students. We are able to bill health insurance for all services that are not prepaid, as well as for diagnostic services such as x-rays and laboratory tests.

We strongly encourage all students to have some form of health insurance. Information on the health plans sponsored by the UW is available at the Student Insurance Office website.

To facilitate your student's care, we ask that your student complete a patient Pre-Registration form and return it to Hall Health. You can obtain the Pre-Registration form from our website at http://depts.washington.edu/hhpccweb/sites/default/files/pre-registration%20form.doc. We also encourage your student to register with us as a patient and use our eCare system. eCare allows us to respond to email messages and send health alerts and other information. You may also want to take time to review the Notice of Privacy Practices.

If your student will be less than 18 years old when he or she comes to campus, a parent or legal guardian must sign the Consent for Care form and return it to HHPCC along with the Pre-Registration form. You can download the Consent for Care form, or, if you do not have access to a printer, please call (206) 616-2495 and we will fax or mail you this form. The Consent for Care form must be on record at Hall Health before we can treat a patient under age 18.

All form(s) should be returned to: Patient Registration, Hall Health Center Box 354410, University of Washington Seattle, Washington 98195-4410

During the 2011-2012 school year, we completed a multiyear renovation project that allows us to better serve our patients, so please come see our new and improved facilities! If you have questions about any of our services, visit our web site or call (206) 616-2495.

Thank you for your cooperation and again, congratulations on your student's acceptance to the UW.

Sincerely,

David C. Dugdale, MD
Director, Hall Health Center
